Athlete Medical Information
New Athletes
All new athletes (those who have never competed for a UVic varsity team) must fill out the mandatory pre-participation Fit to Play medical forms.
PLEASE NOTE: The Fit to Play and Pre-Participation Examination Forms must BOTH be signed by the athlete's physician.
Form Instructions
Fit to Play medical form needs to be filled out emailed to sic@uvic.ca.
- Athlete completes Part 1 Fit to Play History Form - must be signed by the athlete's physician
- Family physician (not clinic practitioner) completes Part 2 Pre-Participation Examination Form - must be signed by the athlete's physician
If the medical is not submitted, the athlete will NOT be permitted to participate in any practice or S&C sessions until the medical is completed.
As we recognize how hard it is to get medical appointments in B.C., at the least athletes must have an appointment date confirmed with a physician, and email sic@uvic.ca with the following declaration:
“I have completed Part 1 “Fit to Play History Form” and booked an appointment with my family physician to complete Part 2 “Pre-Participation Examination Form” on DD-MM-YYYY”.
If an athlete is unable to make an appointment with a physician, connect with the Vikes Sports Injury Clerk (sic@uvic.ca) after completing Part 1 Fit to Play History Form.
